Cuando el juego empieza como tal, nos damos cuenta que fuimos enviados por nuestro padre, un médico cirujano muy respetado en su entorno, para ver a ese mismo padre muerto y quedar nosotros como únicos sospechosos. Luego nadie nos da razón de lo que pasa y arranca Pathologic 2.

Este juego es en esencia los esfuerzos por resolver un caso de asesinato. La muerte del padre del protagonista es quien lleva el hilo de la narración en Pathologic 2 y gracias a eso se logra explorar la ciudad natal del personaje central. A medida que vamos recorriendo dicha ciudad, hablaremos con viejos conocidos que nos aumentarán los enigmas y misterios por descifrar.

¿Qué hay detrás de la plaga apocalíptica que rodea este juego? ¿Por qué los cuervos rodean la ciudad tantas veces? ¿Por qué siempre llega tarde el tren con los suministros? Esa y otras muchas dudas tendrán nuestro tiempo ocupado en Pathologic 2 y será nuestro deber establecer que es lo que ocurre.

La manera que este juego se comunica con nosotros es mediante simbología, mitología oscura, metáforas y raras imágenes del mundo de los sueños.

La trama se compone de nutridos diálogos que guardan información en clave y son cruciales seguirlos para nuestro efectivo progreso. Mientras transcurre la argumentación veremos una serie de imágenes e ilustraciones muy bien hechas.
